Award Recipients (not attending UFV) Claim Form

Please submit this claim form no later than August 15, 2018 for the Fall semester.

Award funds will be mailed in early September to your accredited post-secondary institution to be deposited into your student financial account.

Students who graduated in 2017 have until December 15, 2018 to submit their award claim form or the award(s) will be forfeited.

Students who graduated in 2018 have until December 15, 2019 to submit their award claim form or the award(s) will be forfeited.

 

Award Recipients attending the University of the Fraser Valley (UFV) UFV Claim Form

Please submit this form no later than August 15, 2018 for the Fall semester.

You must also electronically submit a UFV enrolment verification letter dated after September 18, 2018 (this is the last day to withdraw and receive a refund) to info@abbotsfordcf.org.

These two documents must be submitted before October 9, 2018 and then your award monies will be mailed to your local residence.

Students, attending UFV, who graduated in 2017 have until December 15, 2018 to submit their claim form or the award(s) will be forfeited.

Students who graduated in 2018 have until December 15, 2019 to submit their award claim form or the award(s) will be forfeited.

Donor Thank You Letters
All award recipients are required to write a letter of thanks to the donor(s) who funded their award and send it to info@abbotsfordcf.org.  We will then forward the letter to the donor. To help you write your thank you letter see – Sample of a Thank You letter
