Award Recipients (not attending UFV) – Claim Form
Please submit this claim form no later than December 15th, 2017 for the Winter semester.
Award funds will be mailed in early January to your accredited post-secondary institution to be deposited into your student financial account.
Students who graduated in 2016 have until December 15, 2017 to submit their award claim form or the award(s) will be forfeited.
Award Recipients attending the University of the Fraser Valley (UFV) – UFV Claim Form
Please submit this form no later than December 15th, 2017 for the Winter Semester.
You must also electronically submit a UFV enrolment verification letter dated after January 17, 2018 (this is the last day to withdraw and receive a refund). Send this letter to email@example.com.
These two documents must be submitted before February 2, 2018 and then your award monies will be mailed to your local residence.
Students, attending UFV, who graduated in 2016 have until December 15, 2017 to submit their claim form or the award(s) will be forfeited.
Donor Thank You Letters
All award recipients are required to write a letter of thanks to the donor(s) who funded their award and send it to firstname.lastname@example.org. We will then forward the letter to the donor. To help you write your thank you letter see – Sample of a Thank You letter
